Chart Review of E/M CPT Usage:
The Chart Review of E/M CPT® Usage report compares the code used (Practice CPT), the
appropriate code (Documented CPT) and the potential (Optimal CPT) code which is based on the
Medical Decision Level. The Optimal CPT is an indicator that the documentation of patient care
is insufficient to the patient's needs. This report is printed for each physician and with all
physicians summarized on one report. The Chart Review also compares the respective fees for
each of the three categories. The report lists what the Practice Charged, what the Chart
Documented, what E/M CPT would have qualified based on the Medical Decision. CPT, fee and
percentages will be itemized and tallied.
 The Chart
Review of E/M CPT Usage may now be requested in HIPAA Compliant format, providing the same patient data without printing specific patient names and identification numbers.
 If Teaching Physician Guidelines are activated, the Documented CPT field will display "NoTPG" in the case of
incomplete guideline compliance.
